Self Assessment Forms
The Self-Assessment forms must be completed by every junior. It is an eight page document with questions that help your CAC get to know you better. Part of the form is an autobiography you must write for your counselor. More information on the autobiography will be available soon. The second form is a "Reality Test". You'll have to fill this out for a certain section of the Self-Assessment form. Instructions will be given in the form.
