Report an Incident

General

Use this form to report general incidents of misconduct. Incidents include, but are not limited to:

  • Alcohol, drug or tobacco use
  • Safety and security violations
  • Damage to state property
  • Housing policy violations
  • Physical altercation
  • Code of Conduct violations
  • Information Technology Systems policy violations
  • Disruptive behavior

Student of Concern (BIT Care Referral)

Use this form to share non-urgent or non-emergency observed student behavior. It is sent to members of IMSA’s Behavioral Intervention Team (BIT Care). This form does not constitute submitting a police report or a general incident report. For questions about the appropriate time to use this form, please contact the Office of Student Affairs at 630.907-5009.

Bias

Use this form to report a non-urgent incident involving bias. It is sent to the Office of Diversity, Equity and Inclusion.

Academic Misconduct

Use this form to report incidents of academic misconduct. Academic misconduct includes, but is not limited to:

  • Cheating
  • Plagiarism
  • Use of unauthorized sources
  • Prohibited behaviors noted on a class syllabus

Medical Incident

This form is used to report individuals involved in medical or traumatic situations. It should be used anytime someone seeks or needs medical attention, regardless of transport to the hospital.
